New Patients Welcomed at Abbott Foot & Ankle Clinic

Welcome to Abbott Foot & Ankle Clinic! New patients often have questions prior to their first appointment. In order to make your visit stress-free, we’ve provided some helpful tips to ensure that you are prepared. Our friendly staff will make you feel right at home. From the time you enter our convenient and inviting office, until you leave with your foot and ankle questions answered, our goal is to provide excellent podiatric care and personal attention.

What Should I Bring to My Appointment?

Your time is valuable, so we strive to make each visit efficient. You can prepare for your appointment by completing the New Patient form prior to your arrival. This questionnaire is invaluable to Chiropodist Tony Abbott, since the feet often show the first signs of significant health problems. Your medical history and current symptoms are important pieces in the diagnostic puzzle; please take time to fill in the information accurately and completely. Don’t forget to list all current medications (both topical and oral) and any allergies.

As you complete the form, take this opportunity to write down any questions you have for our foot specialist. We believe that patient education is critical and reassuring. We want you to leave our office with the answers to your questions and the tools you need to restore the health of your feet.

Two to three pairs of your current footwear will provide information about your foot mechanics and gait. Bring a variety of types—work shoes, athletic footwear, and your favourite pair of casual shoes as well. The wear and tear patterns of your shoes tell us a lot about your unique feet including important clues that help to determine the source of your pain.

If you wear shoe inserts that you have purchased on your own, or if you have been prescribed medical custom orthotics or foot devices in the past, bring those along as well. Your discomfort could stem from something that was supposed to help. These over-the-counter inserts will be examined to determine if they are helping or hurting your foundation.

Before you arrive for your appointment, check to see if you have the following:

  • New Patient Form

  • 2-3 Pairs of Shoes
  • Orthotics, over-the-counter inserts, or other devices

A thorough discussion of your past medical history, a comprehensive foot evaluation, and diagnostic testing (when necessary) will provide the basis for your individualized treatment plan. Questions? Patient Forms

Below is a link to our New Patient form. Please print and complete it before your appointment date. Doing this at home will make it easier for you to provide an accurate medical history, as well as a list of all oral and topical medications that you use.  Be sure to identify any allergies that you may have.

Bring along any foot orthotics (inserts), off the shelf arch supports or other foot devices that you use or have used in the past.

It is also important that we check your footwear. Please bring 2-3 pairs of shoes that you wear the most including dress, casual, athletic shoes or walking shoes.

  • Clinic will be on the Right, in same plaza as Collingwood Hearing Clinic.  Hyundai dealership will be across the street.

From Thornbury/Clarksburg area:

  • Take ON-26 E
  • Follow into Collingwood
  • Turn L at lights to stay on ON-26 E (Holiday Inn will be on your Left)
  • Follow through Collingwood
  • Turn L onto ON-26 E
  • Clinic will be on Left, in same plaza as Collingwood Hearing Clinic.  Hyundai dealership will be across the street.
